Pediatric Membership Plan Details
- Does not cover procedures provided by specialists outside the practice or independently contracted anesthesia services.
- The membership plan is not insurance, a qualified health plan under the Affordable Care Act, pre-payment for future services, or payment for access to discounted services. Rather a membership plan is a direct payment arrangement between you and your dentist for services, products and care provided.
- The infant membership plan runs from time of purchase to the end of the 2nd birthday month. The child and teen membership plan runs from time of purchase for one calendar year. There are no refunds for unused services.
- Payment for all treatment is due at the time of service.
- The annual fee for the membership plan cannot be paid for with third-party financing such as Care Credit.
